• If you are paying for your own sessions, or if you intend to claim a Medicare or Private Health rebate, click on “I am privately paying for sessions”.

    Once you complete your new client details form, you will be sent to a booking link to secure your session with us. We will then send you an email with your Welcome Pack and a questionnaire to fill out.

    If you are using Medicare funding, please ensure that a GP referral has been completed prior to paying for your session to receive a rebate.

  • Click on the button above that says “I am making an NDIS referral”. Once you complete referral form, we will need to check the referral make sure we are can allocate the best therapist fit.

    We will then send a service agreement via email or text. Once signed, you will be sent an email with a booking link to secure your session with us, along with a questionnaire and your Welcome Pack. Please note that we are unable to accept NDIS referrals for the following:

    -Complex home modifications

    -Mobility assessments and equipment

    -We have fluctuating capacity to complete housing assessments (SIL, SDA). Please contact us before placing a referral to find out our availability.
